Years of Life Lost and Productivity Costs Due to Premature Cancer-Related Mortality in Ukraine and in Kiev City, Lviv, and Cherkasy Regions

OBJECTIVES: This study aims to provide a perspective on the cancer mortality burden and productive life lost in Ukraine, specifically in Kiev city, Lviv and Cherkassy regions.
METHODS: Human capital approach was used to estimate years of life lost (YLL), due to premature death, and present value of future loss of productivity (PVFLP). Mortality data were sourced from the National Cancer Registry of Ukraine. Year of productive life lost (YPLL) measures the number of working years lost prior to the standard retirement age of 60, capturing the impact of premature mortality on economic productivity.
RESULTS: In 2022, 112,244 patients died of cancer in Ukraine, with 2,438, 3,457, 2,274 death in Kiev city, Lviv and Cherkassy, respectively. Melanoma, breast, cervical, endometrial and ovary cancer are malignancies with high mortality rate with estimated numbers of 2,403; 12,335; 3,004; 5,601 and 2,806 deaths in Ukraine in 2022, respectively. An estimated number of deaths in melanoma of 42, 46 and 41; in breast of 271, 262 and 191; in cervical of 55, 75 and 55; in endometrial of 82 ,93 and 49 and in ovary cancer of 64, 87 and 49, in 2022, respectively for Kiev, Lviv and Cherkassy. All cancer deaths resulted in overall 1,393,352 YLL in Ukraine (20,201 in Kiev, 29,197 in Lviv and 17,789 in Cherkassy), with melanoma accounting for 37,769, breast for 243,998, cervical for 78,836, endometrial for 94,139 and ovary for 59,048 YLL in Ukraine in 2022. Annual PVFLP in Ukraine was estimated to be $ 1,742,642,354 for all cancer types, with melanoma accounting for $58,684,143, breast for $239,937,174, cervical for $98,405,881, endometrial for $60,639,269 and ovary cancer for $57,996,124 in 2022, respectively.
CONCLUSIONS: YLL and PVFLP due to premature cancer mortality in Ukraine are substantial and hence continuing prioritization of investment in cancer care is imperative to improve societal burden for Ukraine.
