OBJECTIVES: Broader societal value beyond clinical benefits of innovative therapies remains largely unquantified in China, which is important to assess their overall social impact given the higher medical costs. This study estimated the societal value generated by guideline-compliant HER2-targeted therapies for breast cancer in China from 2021 to 2030.
METHODS: A societal value framework was adapted to estimate health gains and productivity impacts. 6 targeted therapies across neoadjuvant, adjuvant and advanced settings were included. Patients reached were estimated based on real-world utilization from 2021 to 2025 and projections from 2026 to 2030 based on breast cancer prevalence from GBD data. Incremental QALYs were derived from published pharmacoeconomic literatures. Societal value was quantified through productivity gains in paid and unpaid work and monetized using gross value added, with additional value-chain effects estimated using 2023 input-output multipliers. One-way analyses assessed key parameters uncertainty.
RESULTS: Number of patients reached is estimated to increase from 158,906 in 2021 to 226,192 in 2030, with neoadjuvant treatment contributing the largest share. A cumulative gain of 71,202.0 incremental QALYs was estimated, translating into a total societal value of $1.63 billion. Paid work accounted for 76.80% of the total with a value of $1.25 billion. Results were most sensitive to the proportion of HER2 cases among all patients.
CONCLUSIONS: Targeted therapies for HER2-positive breast cancer generated substantial societal value in China. Incorporating a societal perspective may improve alignment between payment decisions of innovative therapies and overall social welfare.
