CONSTRUCTION OF A VALUE ASSESSMENT FRAMEWORK FOR NEW ANTIDIABETIC DRUGS TO SUPPORT EVIDENCE-INFORMED DECISION-MAKING

OBJECTIVES: To develop and apply a multi-criteria value assessment framework for new antidiabetic drugs to support evidence-informed healthcare decision-making.
METHODS: A structured framework was constructed using multi-criteria decision analysis and the measuring attractiveness by a categorical based evaluation technique (MACBETH) approach.Criteria were identified through literature review,focus group discussion and stakeholder consultation.The performance data of antidiabetic drugs was mainly extracted from evidence review.Criterion weights and value functions were derived from stakeholder judgments using M-MACBETH software.The framework was applied to assess the value of six new antidiabetic drugs.Sensitivity and robustness analyses were conducted to examine stability of findings.
RESULTS: The value assessment framework consists four core domains and nine quantitative criteria,involving comparative effectiveness,patient-reported outcomes,safety,and affordability.Based on judgments of criterion attractiveness,changes in hemoglobin A1c (19.33%),weight loss (15.61%),and risk of major adverse cardiovascular events (13.01%) had the highest weight.The piecewise linear value functions were established to convert drug performance on each criterion into value scores for aggregation.Among the six new drugs, Xigduo XR achieved the highest overall score.Xigduo XR showed superior performance in improvement of health-related quality of life and affordability,while iGlarLixi exhibited advantages in reducing the risk of major adverse cardiovascular events.Sensitivity analysis showed the drug ranking remained stable given reasonable variations in criterion weights. Robustness analysis further confirmed the reliability of findings.
CONCLUSIONS: This study developed a transparent MACBETH-based framework for assessing the value of new antidiabetic drugs.By integrating evidence and values,the framework would support evidence-informed healthcare decisions and value-based diabetes care.
