PREVALENCE AND RISK FACTORS OF THROMBOCYTOPENIA IN ICU PATIENTS: A SINGLE-CENTERED STUDY

OBJECTIVES: Thrombocytopenia is a common finding in intensive care unit (ICU) patients associated with poor prognosis and increased mortality rate. Despite its clinical importance, the data from developing countries like India remains limited. So we aim to study the prevalence and risk factors of thrombocytopenia in ICU patients.
METHODS: We conducted a single-center cross-sectional observational study. Patients admitted to the ICU for ≥ 24 h and over 18 years were included in the study. A total of 630 ICU patients were screened for thrombocytopenia (≤150,000/cumm) as well as demographic parameters, complete blood counts, liver and renal functions and APACHE (Acute Physiology and Chronic Health Evaluation)-II score was assessed. In this current study, we also included 149 ICU patients, who don’t have thrombocytopenia as a control group.
RESULTS: Out of 630 patients, 23.65% (n=149, 94 Male & 55 Female) patients had thrombocytopenia. Out of 149 ICU patients, 14.76% patients having increased APACHE-II score compared to the ICU patients who did not having the thrombocytopenia (4.69%) indicates a greater likelihood of development of sepsis. Patients who have thrombocytopenia showed a significant difference (p˂0.05) in diastolic blood pressure, hemoglobin level, serum potassium, creatinine level and liver function tests compared with the non- thrombocytopenic ICU patients. On pearson correlation analysis, a significant correlation found between the RBC, WBC, serum potassium, creatinine level, SGOT, SGPT and strong correlation APACHE-II Score (r=-0.47, p˂0.001).
CONCLUSIONS: This current study results suggest that thrombocytopenia is more prevalent in ICU patients correlated with severity of illness assessed by the APACHE-II score, kidney and liver function, blood cells and serum potassium level. The present study findings suggest that timely platelet monitoring potentially could be dynamic prognostic marker in the ICU setting.
