We welcome the commentary by Ramagopalan questioning why distributional cost-effectiveness analysis (DCEA) has not been more widely adopted in economic evaluations conducted by health technology assessment (HTA)  agencies.  Because reducing health inequalities is a social objective supported by public opinion and commonly recognized by HTA agencies, it is appropriate to periodically examine whether best practice is being enacted.
