The Incidence, Mortality, and Survival of Nonmetastatic Colorectal Cancer (CRC) per TNM Stages in the United States: A Surveillance, Epidemiology, and End Results (SEER) 2018-2022 Database Analysis

OBJECTIVES: Surveillance, Epidemiology, and End Results (SEER; available from April 2025) data were used to investigate the non-metastatic colorectal cancer (CRC) burden in the US.
METHODS: 17 cancer registries (January 2018 - December 2022) were analyzed with SEER*Stat. The target population was non-metastatic CRC (M0 stage). Outcomes were crude incidence and mortality rates (per 100,000 population) and survival.
RESULTS: There were 136,565 non-metastatic CRC patients. Most of them were 65+ years old (56.13%), males (52.79%), non-Hispanic Whites (62.07%), married (50.12%), from metropolitan counties (87.25%), and with a $65,000-90,000 annual household income (44.90%). Per TNM cancer classification, the most frequent stage was T3-N0-M0 (20.43%). The incidence was 31.21/100,000 in the 2018-2022 period. Per TNM, the highest rates were observed for N0 (18.66/100,000) and T3 (12.43/100,000) categories, with the highest individual result for T3-N0-M0 cancers (6.38/100,000). The incidence trends showed a slight increase (31.59/100,000 in 2018 vs. 31.96/100,000 in 2022). The lowest and highest rates were in 2020 (28.16/100,000) and 2019 (32.29/100,000). The mortality was 6.80/100,000 in the 2018-2022 period. Per TNM, the highest rates were reported for N0 (3.18/100,000) and T3 cancers (2.19/100,000). The highest individual rate had the TX-NX-M0 class (1.39/100,000). Mortality trends showed a continuous annual increase between 2018 (2.79/100,000) and 2022 (10.13/100,000). Patients diagnosed in 2018 had 1-year and 4-year survival of 89.97% and 74.00%. The lowest 4-year rates were captured for NX (49.06%) and TX (47.73%, excluding T2b with 0% due to the small sample). In 2021, the 1-year survival was 88.46%. Categories with the lowest results were NX (68.59%) and TX (58.32%, excluding T2a and T2b with 0% due to the small samples).
CONCLUSIONS: The study findings may indicate a high non-metastatic CRC burden in the US. Substantially increasing mortality trends during the last 5 years of data propose a need for new and effective treatment options.
