ISPOR 2019 Short Course Program is Co-Sponsored by ISPOR and Purdue University College of Pharmacy, Office of Continuing Education & Professional Development.

 

For physicians (CME):

Physician Accreditation Statement – This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Purdue University College of Pharmacy and ISPOR. Purdue University College of Pharmacy, an equal access/equal opportunity institution, is accredited by the ACCME to provide continuing medical education for physicians.

Credit Designation Statement – Purdue University College of Pharmacy designates these live educational activities for a maximum total of 16.0 AMA PRA Category 1 Credit(s)TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity. Please see Short Course Summary for individual program credit assignments.

For pharmacists (CPE):

Pharmacist Accreditation Statement – Purdue University College of Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. ***Please note: Short courses are accredited based on presenters' compliance with ACPE regulations. All presenters who comply with ACPE regulations will have their session accredited for Continuing Education credit. Accredited sessions will be designated.

Instructions

To receive continuing education credit for the ISPOR Short Course Program, you must either pre-register online or register onsite.

If you pre-register: Registrants will receive ISPOR 2019 Continuing Education Information and Instructions via email confirmation. Alternatively, a Continuing Education materials ticket will be included in your registration materials. Registrants will be able to redeem this ticket for ISPOR 2019 Continuing Education Information and Instructions at the onsite registration help desk.

On-site registrants: To obtain ISPOR 2019 Continuing Education Information and Instructions, you must register for this service at the ISPOR Registration Help Desk during the conference.

The fee for this service is US$125.

To receive continuing education credits: Follow all instructions provided to complete the online Evaluation/Request for Credit for each session.

 

For CME: Certificates will be sent 4-6 weeks after the completion of the online Evaluation/Request for Credit.

 

For CPE: Pharmacist's ACPE credits will be directly reported to CPE Monitor within 60 days. Paper certificates will not be provided. This requires that the participant provide their NABP eprofile ID and MMDD of birth. If you have any questions regarding the NABP ID please visit http://www.nabp.net/programs/cpe-monitor/cpe-monitor-service/ . You must log into your NABP e-profile to view your certificates.
