PRESENTATION TIME |
| introduction, 12 minutes for presentation and
2 minutes for
questions and discussion. It is important that you do not exceed your
allotted time. |
| Your podium presentation has been grouped
with other presentations of similar content. During your presentation you
are encouraged to highlight aspects of your presentation that are
consistent with the title of your session. |
| See your emailed abstract acceptance
notification or confirmation letter for
the date and time of your podium presentation. When you arrive at the
meeting, please consult the program brochure in your registration
packet to confirm the exact time and location. |
HANDOUTS
All presenters are required to bring handouts of their presentation. It is
suggested that you bring about 100 copies. |
POSTING PODIUM PRESENTATIONS AT THE ISPOR
WEBSITE
To have your podium presentation posted on the ISPOR website, send your
presentation(s) as a presentation file (such as PowerPoint) or a word
document file (such as Word), or as a pdf file to:
presentations@ispor.org. and
complete the Poster/Podium Presentation Release Form found at
www.ispor.org/present_relf.htm.
Fax back the completed release form to 1-609-219-0774.

| Each session room will be equipped with an
LCD projector, laptop computer and screen. Please email your completed
slide presentation in Microsoft PowerPoint (2000 or earlier) to
dklim@ispor.org NO LATER THAN
MONDAY, MAY 3, 2004.
ABSOLUTELY NO CHANGES
WILL BE ACCEPTED AFTER THIS DEADLINE. Your presentation will be loaded and
opened for you when you arrive at the session. |
| 35mm slide projection will NOT be allowed.

| Please arrive in your session room at
least 15 minutes before the session begins to familiarize yourself with
the AV equipment, the moderator and the other speakers. |

MEETING REGISTRATION
Register for Annual Meeting at
https://www.ispor.org. You are required to register for
this Annual Meeting by April 7, 2004 to have your presentation included in the
final Program and Schedule of Events. Please complete the registration form and return with payment
to the ISPOR office, if you have not already done so.
HOTEL RESERVATIONS
The
deadline for hotel reservations is August 19, 2005.
